Funeral directors or other authorized entities must register deaths in Berkeley within ten days.

Health and Safety Code Sections (HSC) 102400 and 102415 require that all deaths are registered with the City of Berkeley, local registrar within ten days of death. Please find Electronic Death Registration System (EDRS) instructions and request to unlock record form below:

You can choose to apply online or by fax or mail.
Online via the Funeral Home Portal
To use the online portal, you must have an account. To request an account for the Funeral Home Portal, please email the following information to fhportalsupport@vitalchek.com:
- Name of Funeral Home
- Full mailing address
- Phone number
- Name of person submitting orders
- The email address for the account
With an online account:
- Funeral Homes can order death certificates through our online portal provided by VitalChek
- A processing fee of $6.00 will apply for payment with credit cards or $2.50 for electronic checks
- Express delivery and will-call pickup are available
