If you’ve called for emergency medical services from the City and were assisted or transported by Berkeley paramedics, you can access available documents and resources.
Get patient care reports and AmBULance billing records
A patient care report (PCR) is generated whenever the Berkeley Fire Department assists a sick or injured person.
Call Wittman Enterprises at (800) 906-6552 for:
- A free digital copy of a PCR for yourself or your dependent, and/or
- A copy of ambulance billing records for yourself, a dependent or a client.
Hard copy of PCR
If you need a PCR in hard copy, download and complete the appropriate request form:
Bring or mail the completed form and $20 fee payment to the Berkeley Fire Department Administration Offices. Checks or money orders can be made payable to "Berkeley Fire Department."

For PCR questions, please contact the Berkeley Fire Department at (510) 981-5595 Monday through Friday from 8:00 am to 5:00 pm, or email BFDEMS@berkeleyca.gov.
Copies of PCRs are available to our patients and/or their authorized representatives. These reports contain confidential medical information and are not part of the public record. These records are kept in compliance with the Health Insurance Portability and Accountability Act (HIPAA).

Call Wittman Enterprises at (800) 906-6552 with any billing or insurance questions about ambulance transport that the Berkeley Fire Department provided.
The Fire Department contracts with Wittman Enterprises to provide billing services for all of our ambulance transports.
File a complaint
If you have a complaint regarding Berkeley Fire Emergency Medical Services, please email BFDEMS@berkeleyca.gov. In your email, please provide:

- Date/time of service
- Nature of your complaint
- Contact email
- Contact phone number, and if this phone is text-capable.

Emergency Medical Services in Berkeley
All of Berkeley’s seven fire engines, two trucks, and four ambulances are staffed with paramedics. Each emergency calls for medical care in the city of Berkeley receives paramedic response from the closest available unit. These units stabilize the scene and patient, begin care, and support/facilitate transport to the hospital via ambulance as needed. Berkeley ambulances transport to hospitals within Alameda County based on patient acuity and County protocols.
The Berkeley Fire Department also oversees emergency medical services for large events, and may provide standby services or require an event to contract EMS services to minimize the event’s impact on the 911 system.
